Overview:
Accountable Care Organizations (ACO) are proliferating in the United States, but providing pastoral care in such a setting is new and untried. This workshop will describe the model that MissionPoint Health Partners, an ACO, has created to provide pastoral care to its members. The presenter will discuss the creation of the pastoral care program, common issues that arise in providing holistic outpatient spiritual care, and the ongoing collaboration between the hospital and ACO chaplaincy departments. We will also discuss different methods of providing pastoral care at a distance to diverse populations.

Presenter:
Rev. Catherine Chang, MDiv BCC is a chaplain at Ascension Care Management (ACM), a Catholic population health company, working to provide spiritual care in an ambulatory setting. Prior to her work at ACM, she was a hospital chaplain at a number of hospitals including St. Thomas Health in Nashville and Johns Hopkins Hospital in Baltimore, MD. Cathy is ordained as a Unitarian Universalist minister, and is a Board Certified Chaplain with APC. She lives in Nashville with her husband, two children, her parents and her border collie mix dog.
